Job Description:
We are seeking a compassionate and organized Care Coordinator to join our team. In this role, you will work closely with clients, their families, and healthcare providers to ensure seamless coordination of care.
Your responsibilities will include:
- Scheduling and organizing carer appointments.
- Managing care plans and patient records
- Managing quality of care through spot checks and Supervisions
- Communicating effectively with healthcare professionals
- Providing support and resources to patients and families
- Ensuring compliance with healthcare regulations
Qualifications and requirements:
- Previous experience as a Care Coordinator preferred, however this could also be an opportunity for an exceptional Carer or Senior Carer (training would be given)
- Excellent organizational and multitasking skills
- Strong communication and interpersonal abilities
- Proficiency with healthcare management software
- A compassionate and empathetic approach to patient care
- Driving licence and own car
