Annual Physical Health Check Co-ordinator for Serious Mental Illness (SMI)

The Role:

GRCC is looking to appoint a countywide Annual Physical Health Check Co-ordinator for Serious
Mental Illness (SMI). People with SMIs are reported as being at greater risk of poor physical
health and die on average 15 to 20 years earlier than the general population. This vital role
works with GP surgeries to empower patients with SMIs to access their free annual physical
health check. Our project has helped numerous individuals to improve their wellbeing and
address underlying health conditions, increasing uptake in appointments to 78% in the last 12
months. We need to do more.

What We Need:

We are looking for a ‘people person’, someone with the patience and understanding to work
with this vulnerable group and help them overcome the barriers to accessing free health
checks. You will liaise with GP surgeries to build strong relationships, work with patients, and
attend health check appointments if needed. You will be able to communicate at all levels, be
proficient in IT (Microsoft Word, Excel, and PowerPoint), and be able to present to a range of
audiences.

Salary: £25,200 per annum (pro rata)
