You will work closely with individuals to develop personalized care plans, offer emotional and practical support, and connect them with essential resources. In this role, you will develop and implement individualized care plans, provide one-on-one support and guidance, facilitate group sessions and workshops, collaborate with multidisciplinary teams to ensure holistic care, and advocate for clients’ needs and rights.
