Would you like to work in Gloucester and Forest of Dean in the community?

Are you passionate about providing excellent nursing care to working age adults? If so we have a post available for permanent full time Band 6 Mental Health Practitioner at Pullman Place working in our Community Assertive Outreach Team. This is a wonderful developmental opportunity.

You will develop and improve your skills and knowledge in the community and be an active member of our multidisciplinary team. You will care coordinate a caseload which will be challenging at times as well as rewarding.

You will develop an understanding about dual diagnosis and severe and enduring mental health illness alongside the multitude of co-existing factors that affect the health and wellbeing of our working age adult service users and their carers and families.

We are looking for an enthusiastic team player with good communication skills and the willingness to learn and develop as well as share their knowledge in this exciting field of mental health. In return we offer you the opportunity to work with a highly skilled and experienced MDT providing support and supervision for you, excellent standards of care for our service users. Please note that the standard working hours for this role are Monday to Friday 9am to 5pm, however flexible working is required to cover bank holidays and weekends on a clinical need led basis.

Main duties of the job

Engage in complex health assessments and plan relevant care in partnership with services users e.g. Specialist assessment of complex nursing needs. Utilising the Care Programme Approach (CPA) and in line with Trust policy, provide specialist care, education and advice to service users (including carers) who are suffering from severe and enduring mental health problem to promote recovery and social inclusion a as care co-ordinator as and when required, including adults at risk and supervision procedures

Take responsibility for the provision of appraisal and supervision of other team members including induction of junior staff members.

Ensure sufficient time is allocated to provide and receive clinical and managerial supervision, to include workload management, focusing on improving quality of care and audit in line with the Government Modernisation Programme, through competent planning and organisational skills

Provide mentorship and preceptorship to junior nurses, nurses in training and other members of the multi-disciplinary team ensuring training objectives are met

Develop, implement and evaluate care plans in conjunction with service users, carers and other professionals which reflect individual needs

Implementing risk assessments and management plans on a daily basis.
