Rehabilitation Pathway Manager - C9327-25-0349

Job summary
This is an exciting opportunity to continue to support the development of the rehabilitation pathway for individuals with mental health difficulties residing in Gloucestershire.
The Community Rehabilitation Team was set up in 2024 as a pilot project and has achieved success in supporting individuals living with psychosis to lead more fulfilling lives. The STAR team look to support individuals both in community and inpatient settings as they oversee a variety of functions, including - receiving and responding to requests for funding for non-commissioned services, Personal Health Budgets & s.117 Aftercare funding requests. They have achieved success in reducing the number of out of area placements in recent years.
The teams are warm in nature and hugely motivated to make a difference. If you're an experienced leader with a passion for service development and an ability to strive towards improving and embedding best practice within the two teams then we'd love to hear from you.
Main duties of the job
As Rehabilitation Pathway Manager you will provide management oversight to the Community Rehabilitation Team and STAR Team. The successful candidate will look to work collaboratively with other key stakeholders to further develop and embed the rehabilitation pathway within Gloucestershire for adults experiencing mental health illnesses.

To that end, we continue to prioritise and invest in our commitment to genuinely becoming a Great Place to Work with consistent top-quartile performance in the annual staff survey and Pulse surveys.
