Specialist Forensic Caseload Manager - D9843-ICB-6027HB

Job summary

This role offers an exciting opportunity to work for Gloucestershire ICB as a Learning Disabilities Forensic Caseload Manager. The post sits within an established Integrated High Needs Support function and the wider Disabilities Commissioning Hub. The successful candidate will work across Gloucestershire footprint supporting people with learning disabilities who are at risk of or who have offended where there is risk of placement breakdown or hospital admission.

The successful candidate will work closely with community team colleagues to provide additional wrap around support where there is a risk of placement breakdown, hospital admission or entering into the criminal justice system.

Support people in their homes as much as possible to prevent hospital admission, detention within criminal justice services and out of area placements.

Provide comprehensive assessment formulation and treatment of offending/forensic behaviour through the utilisation of Trauma informed Positive Behaviour Support, the good lives model and human rights-based approaches.

Create capable environment to effectively support people who are at risk of or who have offended through the provision of training and consultation to families and staff teams.

Enable ongoing engagement in Local Area Emergency Protocol meetings LAEP CTRs and forensic led risk assessment and management meetings MAPPA

Main duties of the job

Ensuring that individuals placed in specialist inpatient provision are in the most clinically appropriate setting, and that this will meet their needs.

Supporting the review process for individuals in specialist placements, ensuring that reviews are carried out regularly and those placements continue to meet the individuals needs.

Provision of objective clinical input to ensure that patients are moved through the clinical pathway, with the aim of discharge and a move to community accommodation wherever possible.

Carrying out a check and challenge role on provider requests for additional support, particularly 1to1, for individuals placed in specialist settings.

Facilitating early planning for discharge back to local services as soon as clinically appropriate.

Liaising with community mental health and LD practitioners, NHS England, inpatient ward staff, social workers, and providers to ensure that everybody is signed up to early planning and working towards discharge as the intended outcome.

Supporting commissioners with expertise around forensic risk and individual case management.

Enabling move-on for people with a learning disability through involvement in the Care and Treatment Reviews and move-on planning processes.

Supporting work around establishing more efficient system flow in mainstream mental health provision.
