Care Co-Ordinator

Rowcroft Medical Centre is exceptionally proud of our pro-active approach to supporting our most vulnerable patients and their families.
Based in the heart of our strong and supportive nursing team are a team of Care Co-ordinators and Health Care Assistants who case manage our most vulnerable patients. Currently we cover frail; palliative patients and those living with medical conditions which impact their ability to care and support themselves on a daily basis.
The role will sit within our large and supportive Nursing Team. Managed by our Lead Nurse the team works closely and co-operatively with our GPs and the broader surgery team. In addition to this we have close links to health care professions working within other organisations in Gloucestershire, local community organisations and our Network Pharmacy and Social Prescribing Team.
This is an exciting and developing role which we believe makes a great contribution to the health and wellbeing of these important patient groups and we are very excited to welcome a new member to this valuable team.

Job summary

Based in the heart of our strong and supportive nursing team are a team of Care Co-ordinators and Health Care Assistants who case manage our most vulnerable patients. Currently covering both frail and palliative patients we are now looking to expand this team to include those living with medical conditions which impact their ability to care and support themselves on a daily basis. This would include people living with learning difficulty, addiction, dementia and other mental health conditions.
The role will sit within our Vulnerable Patients team which is embedded within our large and supportive Nursing Team. Managed by our Lead Nurse the team works closely and co-operatively with our GPs and the broader surgery team. In addition to this we have close links to health care professions working within other organisations in Gloucestershire, local community organisations and our Network Pharmacy and Social Prescribing Team.

Learning and development

All staff will be required to partake in and complete mandatory training as directed. It is an expectation for this post holder to assess their own learning needs and undertake learning as appropriate
The post holder will undertake mentorship for team members and disseminate learning and information gained to other team members to share good practice and inform others about current and future developments (e.g., courses and conferences).
The post holder will provide an educational role to patients, carers, families, and colleagues in an environment that facilitates learning.

Managing information
All staff should use technology and appropriate software as an aid to management in the planning, implementation and monitoring of care and presenting and communicating information.
Data should be reviewed and processed using accurate SNOMED/read codes to ensure easy and accurate information retrieval for monitoring and audit processes.
Service delivery
Staff will be given detailed information during the induction process regarding policy and procedure.
The post holder must adhere to the information contained within the organisation’s policies and regional directives, ensuring protocols are always adhered to.

All staff are required to dress appropriately for their role.
Primary key responsibilities
The following are the core responsibilities of the Care Co-Ordinator. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.
a. Undertaking new patient health checks
b. Facilitating routine and 24-hour BP monitoring, advising patients accordingly
c. Acting as a chaperone as required
d. Conducting ECGs as requested
e. Proactively identify and work with a caseload of people to support their personalised care needs.
f. Work closely with lead GPs and Nurses to ensure vulnerable groups, where necessary, receive annual health and Long-Term Condition checks.
g. Where appropriate and necessary deliver care at a person’s home to include the taking of blood and other health checks.
h. Finding out what matters to each, pull together a plan for their health and care needs and explore options to address these in a single personalised care plan.
i. Help people manage their needs, answering queries and providing them support by linking with other health professionals who are best able to help them.
j. Ensure people have high quality health information both for themselves and within the surgery and broader health organisations to better co-ordinate their care and reduce anxiety caused by the complications of navigating health care.
k. Stay up to date with the needs of the cohort, proactively contacting them when the need arrives.
l. Focus care and support on limiting the risk of a crisis and frustrating which could lead to a reduction of people’s health and wellbeing.
m. Enter information into patient records and into other systems used by the Vulnerable Patients Team to ensure information is kept fully up to date and shared with the team.
n. Supporting the Nursing Team with wound care and immunisations (including flu vaccinations)
o. Undertaking all mandatory training and induction programmes
p. Attending a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.
q. Contributing to public health campaigns (e.g., flu clinics) through advice or direct care, including opportunistic health promotional programmes
r. To always maintain a clean, tidy, and effective working area
Secondary key responsibilities
In addition to the primary responsibilities, the Care Co-Ordinator may be requested to:
a. Demonstrate the ownership of responsibility for personal development, learning and performance.
b. Demonstrate performance of skills and activities to others undertaking similar work
c. Attend relevant annual updates and shared practice learning.
d. Participate in practice audit as directed by the audit lead.
e. Participate in local initiatives to enhance service delivery and patient care.
f. Support and participate in shared learning throughout the PCN.
g. Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.
