Band 6 - Continuing Healthcare (CHC) Nurse - D9843-ICB-6034HB

Job summary
The postholder will be part of the Continuing Health Care CHC team with a good knowledge of the CHC National Framework and relevant discharge pathways.
The ICB CHC Team are being asked to review all patients in the Pathway 2 Beds to determine correct discharge pathways to support discharge flow within localities. The Team are being requested to support the actual assessments of people in these beds as required and within short time frames 28 days, to promote discharges and reduce hospital stays.
The successful applicant will take part in local, regional and national initiatives and the nature of the role will be determined by service specifications.
Main duties of the job
The Band 6 Nurse will support with the joint working between health, social care, ICB partners, commissioners, families, and patients, to support patient flow of the Pathway 2 beds.
This is a dynamic role that requires collaborative working with our colleagues across the D2A pathway and teams/agencies within Gloucestershire. The role will support managing timely discharges into community settings with close joint working with our partners in Social Care.
The postholder will work with the Team Lead undertaking delegating tasks and assessments, be knowledgeable of the D2A model and be skilled in the assessments of adults and identifying care and support needs.
The postholder will also have some responsibility for team performance and service developments / improvements, demonstrate initiative and be creative in finding solutions to problems.
To implement the National Framework for Continuing Healthcare CHC and Funded Nursing Care FNC in a timely, efficient and transparent manner and in conjunction with the individual /family and Multidisciplinary Team.
To provide high quality holistic assessments and reassessments to enable the application of the CHC National Framework and to identify the level of care needs and to make recommendations for the commissioning of care.
To support the joint working with all organisations in relation to the relevant hospital discharge pathways under the discharge to assess model pathways.
