Job summary
o work with allocated individuals who have social care needs and serious and enduring mental health difficulties.
o develop and provide social care/health evidence based interventions for individuals of working age with mental health problems who are receiving social care/health funding and managed through the Social Care Hub.
o ensure a clear focus on recovery and social inclusion.
o support and supervise both registered and non-registered staff in delivering care.
The role will entail assessment, support planning and review of social care packages in robust and timely manner, in accordance with The Care Act 2014 requirements.
Main duties of the job
The social worker will advise on or directly undertake the:
- o Care Act Assessments
- o Requests for Tribunal Reports for unknown cases from Wotton Lawn/Charlton Lane/Recovery Units and people who are from out of county
- o Reviews of individuals that Gloucestershire have placed out of county
- o Section 42 enquiries
- o Reviews of accommodation cases
- o Provide a multi-organisational crisis contingency and relapse planning function
- o Be the link for those that Gloucestershire has a Section 117 responsibility for, but reside outside Gloucestershire
- o Undertake the interface social care transition work for young people moving into adult services.
o Provision of case reports when required (e.g. for courts, mental health review tribunals / managers hearings, case reviews, enquiries etc) and attending case discussions, conferences and reviews as required including those integral to adult and child protection procedures.
o Provision of the statutory role of social supervisor to patients under Department of Justice conditions and restrictions, liaising with the travelling to special hospitals and other placements as required.
